Constellation Consulting is based in Albuquerque, New Mexico, USA and was formed in 2018. Our clients include the State of New Mexico's Department of Health, Department of Transportation, Public Education Department, and others. We specialize in projects that allow our team to utilize their strengths, skills, and expertise, to build collaborations and implement positive change in our communities. Our work includes: community program development and implementation; community based organization engagement; continuous quality improvement and systems change work; data collection, strategic planning, logic model and work plan development; conference and event management; convening work groups and community collaboratives; session facilitation and custom training development; support for clinical providers including improving HEDIS measures, patient satisfaction, service expansion, and credentialing; and advocacy, policy, and lobbying activities.
